Identical twins eat in a diner, excited about their plans to open a medical centre. Beverly wants to improve women’s experiences of birth, and Elliot’s all about scientific research. They’re interrupted when Larry the creep asks for a threesome. Both twins cut him down to size, Beverly sharp and Elliot playing mind games.
Later Beverly sits on the toilet, wiping away blood. She scoops a mass from the bowl and says hello. Elliot consoles her in the morning as they walk through the streets of New York, stopping to give a homeless woman her designer shades. At hospital they swap patients, Beverly taking the scary one, Elliot the emotional.
There’s a grisly montage of birth. Afterwards Elliot takes her sister’s blood to test. And does lines of coke in the bathroom. The twins hit the club, Elliot dancing wildly while Beverly works on her phone. Elliot sneaks off for a hook-up. And Beverly interrupts when they’re paged.
At hospital Joseph encourages them to find wealthy donors for the birthing centre. Elliot has doubts – she knows the Parker family’s role in the opioid crisis won’t mesh with her sister’s morals. But Beverly wants her centre at all costs.
She tends to her next patient, a surrogate mother that has carried five children for a rich white woman who attempts to take over the appointment. But Beverly puts her patient first. She’s appalled by Lara’s suggestion that Nicki get pregnant again right after this baby, angry with the exploitation. But Elliot is on board.
Later Elliot scans Beverly, confirming the miscarriage. She’s excited by genetic engineering and designer babies. And very enthusiastic about cocaine. Elliot sniffs more lines, falling into an old argument with her sister. Beverly is crushed by her loss. And Elliot promises to help her have a baby.
In the lab Elliot talks with Tom, gloating about her new centre. She wants to grow embryos to 8 or 9 weeks before implanting. As a scientist, Tom has doubts. It’s 100% illegal. Later Elliot checks on a patient with a yeast infection. While the pregnant woman goes to the bathroom, Elliot has the husband flash his dick – and calls it disappointing.
Beverly smokes out of a window and goes to her next appointment. While examining Genevieve, an actress who longs to get pregnant, Beverly freaks out. She texts Elliot asking for a swap.
They trade clothes, hairstyles, mannerisms… and patients. Elliot knows it’s because her sister’s hot for Genevieve. The actress has major fertility issues. Elliot tries to discuss options, but Genevieve just wants to go to a bar and get drunk. Elliot flirts with her. And the twins swap back.
In the afternoon Beverly deals with two tragic deaths, which could have been avoided with better medical care earlier on. She’s bruised by the loss and determined to improve things with her centre. Elliot offers to seduce Genevieve to cheer her sister up.
Here’s where it gets twisted. Elliot goes to the bar as Beverly, kissing Genevieve and asking her on a proper date. She shoots her shot. And on her way out, Genevieve shares the hotel where she’ll be staying later. Elliot snorts more coke on their way to dinner with the Parkers, revealing that she’s got the actress for Beverly. And Beverly is grateful.
Ruthless billionaire Rebecca Parker and her fourth wife Susan meet the Mantle twins in a restaurant. Rebecca couldn’t care less about the birthing centre. Medical innovation is her thing. Only when Beverly goes off script and pushes back does Rebecca get curious. She invites the twins to spend more time with her.
Afterwards, Elliot returns to her lab and fertilises an egg. Beverly goes to Genevieve’s hotel. She ignores her sister’s calls, listening to Genevieve speak about her longing for a baby. Beverly opens up about her own miscarriages, and they click, kissing at the bar. Beverly brings Genevieve home. They share a meal and have sex, Beverly eating Genevieve out.
Elliot gets home from the lab, sniffing Genevieve’s clothes and panties. She crawls along the table eating their leftover food. In the morning Genevieve’s freaked out by their maid, Greta – and realising Beverly has an identical twin. On the way out, Genevieve asks point blank if they switched. And Beverly lies. They kiss.
The twins return to the diner for breakfast. Elliot offers to seduce the waitress for her, but Beverly refuses. She’s going to see Genevieve again. Elliot isn’t happy. She’s even angrier when Beverly refuses to share details, claiming it’s none of Elliot’s business.
At a grief group, Beverly’s invited to speak. She talks about her twin sister having died. Alone on the toilet, Elliot screams, furious at being shut out.
